29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms. a) On or about October 6, 2020, at the west side of the residential apartment building, the corner end side of the second level of the supported scaffold used by employees to clean stucco on the wall had no guardrail system installed, exposing the employees to a 13.3 foot fall hazard.

29 CFR 1926.451(g)(4)(xv): Crossbracing used in place of a midrail was unacceptable because the crossing point of two braces is between 20 inches (0.5 m) and 30 inches ((0.8 M) above the work platform or as a toprail when the crossing point of two braces is not between 20 inches (0.5 m) and 30 inches (0.8 m) above the work platform or as a toprail when the crossing point of two braces is between 38 inches (0.97 m) and 48 inches (1.3 m) above the work platform. The endpoints at each upright were more than 48 inches (1.3 m) apart. a) On or about October 6, 2020, at the west side of the residential apartment building, the third level corner end side of the supported scaffold used by employees to clean stucco off the wall was missing guard rail components, exposing the employees to a 20 foot fall hazard. b) On or about October 6, 2020, at the west side of the residential apartment building, the middle column second level of the supported scaffold used by employees to clean stucco off the wall was missing guard rail components, exposing the employees to a 13.3 foot fall hazard.

29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards. a) On or about October 6, 2020, at the west side of the residential apartment building, employees working on a supported scaffold while cleaning stucco on the exterior wall had not been trained on the hazards of the scaffold system.
