29 CFR  1910.101(b): "Compressed gases." The in-plant handling, storage, and utilization of all compressed gases in cylinders, portable tanks, rail tankcars, or motor vehicle cargo tanks shall be in accordance with Compressed Gas Association Pamphlet P-1-1965, which is incorporated by reference as specified in Sec. 1910.6. Section 3.3.1 of the CGA Pamphlet P-1-1965 refers to storing cylinders in accordance with CGA and NFPA standards. The CGA Pamplet P-1-1991 Section 3.7.4.1 states that "All compressed gas cylinders in service or in storage shall be secured to prevent from falling."  Native American Investors/Oasis Mobile Home Park, Maintenance-Storage Field: Next to a C-Train/Conex Box a Green Oxygen Cylinder was upright and unsecure from displacement.  Employees were exposed to Struck-by hazards and the potential for airborne projectile.

29 CFR  1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator was competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l):  Native American Investors/Oasis Mobile Home Park, Maintenance-Storage Field:  The employer did not ensure all forklift operators were trained and evaluated in the operation of powered industrial trucks in that the operators were not trained in the use of the Hyster or Powershift type forklifts prior to their use.  This condition exposed the employees to a struck by hazard.

Employees operate Genie and Hyster Forklift(s) to move trash and materials. This condition exposed the employees to a struck by hazard.

On 08/26/2020 an employee allowed a resident from the mobile home park to ride the Blue Genie Forklift resulting in hospitalization of the resident.  Employee(s) were exposed to struck-by, ejection and crushing hazards.

29 CFR  1910.178(p)(1):Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been taken out of service until restored to safe operating condition(s)  Native American Investors/Oasis Mobile Home Park, Maintenance-Storage Field:  The employer did not ensure the Yellow Hyster forklift had been maintained in a safe operable condition. The forklift had clips for seat belt straps and buckle but the straps and buckle were missing.  This condition exposed the employees to ejection and crushing hazards.

The employer  had not ensured the Yellow Hyster Forklift was maintained with the manufactures capacity, operation, and maintenance instruction nameplates, tags, or decals that were legible and/or changed accordingly.  This condition exposed the employees to struck-by, crushing hazards.
