Oasis Mobile Home Park: On and/or before April 20, 2023, employee(s) used a Genie Model GTH 5519 to move pallets of water from a delivery truck to the loading dock of La Chicanita Market. Employee(s) had only completed the online portion of the forklift training, did not receive an evaluation of the operators performance in the workplace, and no practical training was provided. Employee(s) were exposed to struck by hazard(s).
