29 CFR  1910.101(b):"Compressed gases." The in-plant handling, storage, and utilization of all compressed gases in cylinders, portable tanks, rail tankcars, or motor vehicle cargo tanks shall be in accordance with Compressed Gas Association Pamphlet P-1-1965, which is incorporated by reference as specified in Sec. 1910.6. Section 3.3.1 of the CGA Pamphlet P-1-1965 refers to storing cylinders in accordance with CGA and NFPA standards. The CGA Pamplet P-1-1991 Section 3.7.4.1 states that "All compressed gas cylinders in service or in storage shall be secured to prevent from falling."  Indian Trading Post /La Chicanita Market, Inside store: Behind the meat/vegetables display a compressed Helium cylinder was upright and unsecure from displacement.  The cylinder was within a foot of live electrical wires and was used for a coat rack.   Employees were exposed to struck- by and electrical shock hazards.

Employees were exposed to electrical hazard(s).

Indian Trading Post/La Chicanita Market, Inside store main room: Along the Northwest isle way multiple freezers and/or coolers were plugged into extension cords. Employee(s) were exposed to electrical hazards.  a) The Aeolus Air Conditioner was plugged into an extension cord then into an outlet located  at the East wall dock entryway.   b) Four Unilever Island freezer(s) running parallel to the Northwest isle way were found to be plugged into extension cords that were run from the ceiling tile to an unknown termination point.   c) Coke machine cooler type fridge was found to be plugged into an extension cord that had been run from the ceiling tile to an unknown termination point.

29 CFR  1910.303(g)(1)(ii) The required working space about electric equipment rated 600 volts, nominal, or less to ground, was used for storage.  Indian Trading Post/La Chicanita Market, retail store floors: Employees and venders had placed product and supplies directly in front of multiple electrical panels:    a)  The Northeast electrical panel behind the meat display had a black storage rack of supplies, cleaning supplies, and boxes of water stored on the floor blocking access. b)  The East storage room electrical panel had restock beverages placed on the floor in front blocking access. c)   To the left of the Northwest freezers the electrical panel was blocked by  a trash can, boxes, and beverages blocking access. d)  The West storage room electrical panel was blocked by boxes, black trash bag of supplies, and kitchen equipment blocking access    This condition prevented access and immediate shutdown of a circuit to the panel, particularly in the event of an emergency.  This condition exposed employee(s) to an electrical and/or fire hazard(s).

29 CFR  1910.305(b)(2)(i): Each outlet box in completed installations did not have a cover, faceplate, or fixture canopy:   Indian Trading Post /La Chicanita Market, Inside store: In the following locations electrical outlet and switch covers were open showing exposed live wiring. Employees in the process of conducting assigned tasks throughout the market would use various light switches and outlets for lighting and electrical needs.  a) Northwest wall left to the managers office the box enclosure was missing exposing live electrical wiring. b) Northeast wall,  metal butcher table two outlets had broken covers exposing live electrical wiring.  c) Southeast Meat cutting room box cover missing behind the sink, box covers missing on the Southeast wall, the South wall and the light switch located on the west wall had a missing cover showing live exposed wiring.  d) Southwest wall had a missing outlet cover with a slush machine plugged into it. e) Southwest wall light switch just left of the coke fridge had a cover missing with exposed wiring. f)  Northwest Storage room light switch and wall box enclosure had missing covers with live exposed wiring.  Employees were exposed to electrical hazards.

29 CFR  1910.305(d)(2):Panel boards were not mounted in cabinets, cutout boxes, or enclosures designed for the purpose and/or were not dead front:  Indian Trading Post/La Chicanita Market, Inside store: The electrical panel located in the Northwest storage room had multiple missing dead fronts exposing the bus bars, breakers, and live unattached wires. Employees accessed the storage room daily to retrieve and store supplies. Employees were exposed to electrical hazards.
