29 CFR 1910.28(b)(3)(i): The employer did not ensure that each employee was protected from falling through any hole that was 4 feet (1.2 m) or more above a lower level:   (a)  SIRD EAST SIDE SUB-FLOOR ACCESS AREA - Employees performing set-up work for the SIRD lubrication system were exposed to a nine foot nine inch fall hazard into an approximate five foot by five foot void that had been created from the removal of a storage tank for the lubrication system, on or about October 2, 2020.  (b)  SIRD SOUTH SIDE SUB-FLOOR ACCESS AREA - Employees performing set-up work, as well as routine maintenance tasks for the SIRD lubrication system, were exposed to a nine foot nine inch fall hazard into an approximate four foot by five foot void in the walk way, on or about October 2, 2020.  Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
