29 CFR 1910.25(b)(1): The employer did not ensure handrails, stair rail systems, and guardrail systems were provided in accordance with 29 CFR 1910.28.   Maintenance Shop:  Stairs with at least fifteen risers were not equipped with a standard railing on the right side ascending and did not have a mid-rail on the left side ascending.  Employees were exposed to fall hazards.

29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that was 4 feet (1.2 m) or more above a lower level was protected from falling by one or more of the following:  Guardrail systems, safety net systems, or personal fall arrest systems:     Maintenance Shop, second floor storage:  The walking surface above the shop floor did not have an adequate guard rail adjacent to the stair landing; the mid-rail and toe board was missing.  Employees were exposed to falls from heights exceeding 4 feet.
