29 CFR  1926.451(c)(2)(iv):  Front-end loaders and similar pieces of equipment were used to support scaffold platforms without being specifically designed by the manufacturer for such use:  On or about October 22, 2020, while in the process of sheathing and framing at a residential property located at 2727 Carlton St. NW, North Canton, Ohio, employees were using a JLG Telehandler as an elevated working platform.  Employees were working from a height of approximately 14 feet, using a lift that the manufacturer has not approved for lifting personnel, and the employer did not ensure employees were protected from a fall hazard.

29 CFR  1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:  The employer did not protect employees from fall hazards while performing framing and sheathing  on a residential home. A JLG Telehandler, with lumber stacked upon its forks, was extended to the roof line, and used as a scaffold. Employees standing upon the platform of the scaffold were exposed to a fall of 14 feet with no form of fall protection in place.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  The employer does not protect each employee from fall hazards while performing framing activities on a residential home with unprotected edges at a height of approximately 14 feet above a lower level by the use of guard rail systems, safety net systems, or personal fall arrest systems.  This most recently occurred on October 22, 2020, at 2727 Carlton St. NW, North Canton, Ohio 44720.  Employees are exposed to the hazard of falling approximately 14 feet to the ground, while involved in framing activities along the unprotected edges of a residential home without using fall protection or a fall protection system.     Pursuant to 29 CFR 1903.19 the employer must submit documents describing the steps it is taking to ensure compliance, including an explanation of how these steps protect its employees from fall hazards.
