29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):  In the Old Checkers Building, employees engaged in pallet manufacturing activities, were exposed to amputation hazards at the Vertical Bandsaw was used to cut 2 inch thick foam which had approximately 9 ? inch of unguarded blade at the point of operation.

29 CFR 1910.303(g)(2)(i): Except as elsewhere required or permitted by Subpart S of Part 1910, live parts of electric equipment operating  at 50 volts or more were not guarded against accidental contact by use of approved cabinets or other forms of approved enclosures or by any of the means identified in paragraphs (A), (B), (C), and (D) of 29 CFR 1910.303(g)(2)(i):  At the Old Checkers Building, employees engaged in pallet making activities were exposed to electrical shock in that the electrical panel leading to the loading dock was missing the interior cover.
