29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  a)  Smith and Young Roofing, LLC at the jobsite located at 517 West Main Street Bellevue, Ohio; On or about October 22, 2020 the employer did not ensure that employees were protected from head injury hazards by protective hard hats while working below employees who were conducting roofing work above. An employee was exposed to struck by hazards in that an employee on the lower level was working below employees tearing off roofing material without head protection.

Employees were exposed to fall hazards of approximately 8 feet in that a combination ladder in the stepladder configuration was being used to access the second level of a story residential home

29 CFR 1926.1060(a): The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would enable each employee to recognize hazards related to ladders and stairways and train each employee in the procedures to be followed to minimize these hazards:  a) Smith and Young Roofing, LLC at the jobsite located at 517 West Main Street Bellevue, Ohio; On or about October 22, 2020 the employer did not ensure employees were adequately trained to recognize hazards related to ladders and the procedures to follow to minimize these hazards. Employees were exposed fall of approximately eight feet in that a combination ladder in a step ladder configuration was used to access the second level of a residential home from the first level.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a) Smith and Young Roofing, LLC at the jobsite located at 517 West Main Street Bellevue, Ohio; On or about October 22, 2020 the employer did not ensure employees were adequately protected from fall hazards while tearing off roofing material of a residential home. Employees were exposed to fall hazards associated with falls up to approximately twenty-two feet to the ground below.   Smith and Young Roofing LLC, was previously cited for a violation of this occupational safety and health standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1233453, citation number 01, item number 03 and was affirmed as a final order on October 18, 2017 with respect to a workplace located at 7 Queen Anne Way in Norwalk, Ohio 44857

Employees were exposed to fall hazards associated with falls up to approximately twenty-two feet to the ground below.  Smith and Young Roofing LLC, was previously cited for a violation of this occupational safety and health standard 29 CFR 1926.503(a)(1), which was contained in OSHA inspection number 1233453, citation number 01, item number 04 and was affirmed as a final order on October 18, 2017 with respect to a workplace located at 7 Queen Anne Way in Norwalk, Ohio 44857
