29 CFR  1910.133(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:   a) Mixing Area - Employees worked with corrosive chemicals, including, but not limited to, ammonium hydroxide and the employer did not ensure employees used chemical splash goggles exposed to liquid chemicals, on or about October 27, 2020.  Abatement Certification Required.

29 CFR  1910.134(d)(3)(iii)(B)(2): The employer did not implement a change schedule for canisters and cartridges that was based on objective information or data that would ensure that canisters and cartridges were changed before the end of their service life:  a) Mixing Area - Employees were required to wear a half face elastomeric respirator daily when exposed to ammonium hydroxide and a cartridge change out schedule had not been developed, on or about October 27, 2020.   b) Mixing Area - Employees were required to wear a half face elastomeric respirator daily when exposed to silica and a cartridge change out schedule had not been developed, on or about October 27, 2020.   Abatement Certification Required.

29 CFR  1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency  use:  a) Mixing Area - Employees worked with corrosive chemicals including, but not limited to, ammonium hydroxide solution and an appropriate emergency eye wash was not available in the work area, on or about October 27, 2020.    Corrected during inspection.
