29 CFR 1910.178(a)(4): Modifications or additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval: On or about October 3, 2023, and at times prior, maintenance employees were exposed to fall hazards when forklift attachments, which affected the capacity and/or safe operation of the Komatsu Model: FG25T-16 forklift, were used and the employer had not obtained written manufacturer approval for additions. The employer was utilizing an AIG Model series FWP to raise employees to perform facility maintenance.

29 CFR 1910.178(a)(5): The powered industrial truck was equipped with front-end attachments other than factory installed attachments, however the employer did not request that the truck be marked to identify the attachments and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered: On or about October 3, 2023, and at times prior, maintenance employees were exposed to falls when working from a work platform and the employer failed to ensure the Komatsu forklift Model: FG25T-16 was not marked to identify the combined weight of the truck and fork mounted work platform at maximum elevation, with its load centered.
