29 CFR  1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:  On or about 11/6/2020 at building 3000 where two workers were observed exposed to 9-foot fall hazards to the lower level during framing operations. The workers were operating a JLG 660 SJ aerial lift, S/N 10135260 without connecting to the basket.

29 CFR  1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501(b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:  On or about 11/6/2020 at building 3000 where a worker was observed exposed to 50-foot fall hazards to the lower level during roofing operations. The worker was walking the 5/12 pitch, roofing surface without wearing fall protection.

29 CFR  1926.403(b)(2):Listed, labeled, or certified equipment was not installed and used in accordance with instructions included in the listing, labeling, or certification:  On or about 11/6/2020, throughout the jobsite, employees using extension cord plugged together to power electrical hand tools such as but not limited to air compressor and reciprocal saws were exposed to electrical hazards up to 120 volts, in the employer did not ensure the extension cords were used in its intended application.

29 CFR  1926.404(f)(6):The path to ground from circuits, equipment, or enclosures was not permanent and continuous:  On or about 11/6/2020 at 2190 Northlake Parkway Tucker, GA employees were using equipment, which did not have a grounding path. In three instances, the path to ground from equipment was not permanent, continuous, and effective. The following equipment was missing a path to ground: a. on the third floor, the red extension cord powering the Milwaukee reciprocal saw was missing the grounding pin from the plug; b. on the second floor, at panel 75-157, the yellow extension cord missing grounding pin from the plug; c. on the second floor, at panel 75-157, the orange extension cord missing grounding pin from the plug.

29 CFR  1926.405(g)(2)(iv):Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws:  On or about 11/6/2020 at 2190 Northlake Parkway Tucker, GA employees were exposed to electrical hazards up to 120 volts where the employer failed to ensure flexible cords were providing sufficient strain relief.  Workers were observed using equipment that did not have effective strain relief at locations including but not limited to the use of :   a. Yellow extension cord powered by yellow three way splitter extension cord; b. Yellow extension cord plugged into panel 75-157; c.
