29 CFR  1926.503(a)(1):The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  (a) North Peak, Inc. at 1018 Ranch View Rd., Grand Junction, CO: On and about November 16, 2020, the exposing employer did not ensure that employees were protected from injury, in that employees were not trained to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards. Three employees, conducting framing work on the roof of a residential home, had received no fall protection training and were not utilizing a fall protection system. This condition exposed the employees to a fall of approximately 21.5 feet.  Abatement Note: The employer shall assure that each employee has been trained, as necessary, by a competent person qualified in the following areas: 1) The nature of fall hazards in the work area; 2) The correct procedures for erecting, maintaining, disassembling, and inspecting the fall protection systems to be used; 3) The use and operation of guardrail systems, personal fall arrest systems, safety net systems, warning line systems, safety monitoring systems, controlled access zones, and other protection to be used;  4) The role of each employee in the safety monitoring system when this system is used; 5) The limitations on the use of mechanical equipment during the performance of roofing work on low-sloped roofs; 6) The correct procedures for the handling and storage of equipment and materials and the erection of overhead protection; and 7) The role of employees in fall protection plans; 8) The standards contained in this subpart.

29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  (a) North Peak, Inc. at 1018 Ranch View Rd., Grand Junction, CO: On and around November 14, 2020, the exposing employer did not ensure that three employees were protected from injury, in that three employees, conducting framing work on the roof of a residential home, were not utilizing a fall protection system. This condition exposed the employees to a fall height up to approximately 21.5 feet.  Abatement Note: North Peak, Inc. was previously cited for a violation of the same or substantially similar condition of this Occupational Safety and Health Standard 29 CFR 1926.501(b)(13) in the last five years which was contained in OSHA Inspection Number 1420165, Citation 2 Item 1 issued as a repeat-serious citation on August 13, 2019 and became Final Order on September 9, 2019 with respect to a workplace located at 388 W. Ridges Blvd. GRAND JUNCTION, CO 81507.
