29 CFR 1910.184(c)(1): Sling(s) that were damaged or defective were used: On or about November 19, 2020, and at times prior thereto, in the Fabrication Department, the employer did not ensure alloy steel chain hooks used to lift heavy loads were equipped with the manufacturer provided safety latches and replaced the latches when damaged or defective. Employees were exposed to struck-by hazards. 1. Skid 11, alloy chain sling hooks were not equipped with safety latches; and 2. Skid 12, alloy chain sling hooks were not equipped with safety latches.
