Main Street, Hutchinson, Kansas. The employee(s) were in the process of removing the old barrel tube from Thermutator 1 when an unexpectant release of ammonia gas engulfed the room exposing employee(s) to burn / inhalation hazards.

29 CFR 1910.134(c)(1):A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use: The employer failed to implement the respiratory program for an employee wearing a full face respirator to assist in an ammonia line break of Thermutators 1, 2, 3, and 4 in the Taco room located at 512 S. Main Street, Hutchinson, Kansas. During the ammonia line break of Thermutator 1, ammonia gas was unexpectantly released engulfing the Taco room and exposing the employees to burn / inhalation hazards.

Main Street, Hutchinson, Kansas. The full face respirators being worn by the two employees became saturated with ammonia gas during an unexpected ammonia release that engulfed the Taco room.
