(a) On or about July 23, 2025, Piping was routed through the machinery room wall with an unsealed penetration that did not maintain tight construction of the machinery room, without documentation that the room complied with the employer's selected RAGAGEP, IIAR 2 (2021), Standard for Design of Safe Closed-Circuit Ammonia Refrigeration Systems, Section 6.6.2; and  (b) On or about July 23, 2025, Conduits were routed through the louver in the machinery room without documentation that the room complied with the employer's chosen RAGAGEP, IIAR 2 (2021) Standard for Safe Design of Closed Circuit Ammonia Refrigeration Systems, Sections 6.14.5.4.
