29 CFR 1910.213(d)(1): Circular handfed crosscut table saw(s) were not guarded by an automatically adjusting hood which completely enclosed that portion of the saw above the table and above the material being cut: a) On or about November 19, 2020, at the fabrication shop; the self-adjustable blade guard of the Ridgid-12 in. Dual Bevel Miter Saw, Model: R4123 (120V and 4000/min (RPM)) and S/N: GW19272D2A20584, did not cover the blade entirely when the tool was withdrawn from the stock, exposing operators to struck-by hazards created by turning blade causing lacerations, cuts and / or, amputations.

29 CFR 1910.37(a)(3): Exit route(s) were not kept free and unobstructed: a) On or about December 1, 2020, at the fabrication shop, the north-west emergency exit route was obstructed by palletized shingles, trash containers and other palletized materials, preventing prompt access to the emergency exit door. b) On or about December 1, 2020, at the fabrication shop, the south-west emergency exit route was obstructed by boxes, tools and other materials, preventing prompt access to the emergency exit door.
