29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:   a. On or about 12/15/2020, at 4260 Spoleto Circle Oviedo, FL 32765 - the rails of the ladder used to access the roof of a three story residential building did not extend at least 3 foot above the upper landing surface, exposing the employees to a 26 feet 7 inch fall hazard.

On or about 12/15/2020, at 4272 Spoleto Circle Oviedo, FL 32765 - an employee was climbing a ladder holding a five gallon of bucket plastic cement on one hand and a cushion on the other hand, exposing the employee to a 26 feet 7 inch fall hazard.

29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  a. On or about 12/09/2020, at 4266 Spoleto Circle - an employee conducting roofing work from a 6:12 pitched roof of a three-story apartment building exposed to a 26 foot 7 inch fall hazard, in that, fall protection was not used.   b. On or about 12/15/2020, at 4272 Spoleto Circle - employees conducting re-roofing work were exposed to a 26-foot, 7-inch fall hazard from a 6:12 pitched roof, in that employees were working on the roof without being connected to a lifeline attached to an anchorage point.  SUN COAST ROOFING SERVICES INC. WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD 29 CFR 1926.501(B)(13), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 1334706, CITATION NUMBER 01, ITEM NUMBER 001 AND WAS AFFIRMED AS A FINAL ORDER ON 03/08/2019, WITH RESPECT TO A WORKPLACE LOCATED AT 8211 NATURE COVE WAY, TAMPA, FL 33646.  SUN COAST ROOFING SERVICES INC. WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD 29 CFR 19260501 (B)(04)(I), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 1254294, CITATION NUMBER 01, ITEM NUMBER 001A AND WAS AFFIRMED AS A FINAL ORDER ON 10/23/2017, WITH RESPECT TO A WORKPLACE LOCATED AT 4628 TREE TOP LANE, EDGEWATER, FL 32141.
