29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms before being released for use by employees other than erection/dismantling crews: On or about December 10, 2020, workers were laying stone on the residential apartment building from a tubular welded scaffold up to approximately 14 feet high without guard rails, exposing workers to a fall hazard.

29 CFR 1926.451(h)(2)(ii): A toeboard was not erected along the edge of platforms more than 10 feet (3.1 m) above lower levels for a distance sufficient to protect employees below: On or about December 10, 2020, workers were laying stone on the apartments under construction up to approximately 14 feet high on a tubular welded scaffold that did not include toeboards on all working platforms, exposing workers to the hazard of being struck-by tools and materials.
