29 CFR 1926.451(c)(2)(iv):Front-end loaders and similar pieces of equipment shall not be used to support scaffold platforms unless they have been specifically designed by the manufacturer for such use. a) Luis Perez, at a jobsite located at 7273 Quail View Drive, Lot 1244, Sunbury, Ohio 43074 : On and/or preceding December 15, 2020, the employer did not ensure scaffold platforms were designed by the forklift manufacturer to support personnel. An employee was working from an elevated stack of sheathing that was supported by a telehandler forklift, thereby being exposed to a fall hazard of 12 feet, 7 inches.

Each employee engaged in activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure. a) Luis Perez, at a jobsite located at 7273 Quail View Drive, Lot 1244, Sunbury, Ohio 43074 : On and/or preceding December 15, 2020, the employer did not ensure employees were protected from falls while engaged in residential construction activities. Employee were installing roof trusses to a new construction two-story house without any means of fall protection, thereby being exposed to a fall hazard of up to 20 feet.
