29 CFR  1926.20(b)(1): The employer did not initiate and maintain such programs as may be necessary to comply with this part.  a)	Vega Renovations LLC on or about December 12, 2020, at the site location of 2024 Collingwood Toledo, OH: The employer did not have a plan or program to identify job site hazards which assists employees in maintaining a safe working environment.  Such programs would support the employer and employees in identifying and avoiding potential hazards, such as falls during job site operations, decreasing the probability of employee injury and/or illness.

29 CFR  1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards.  a)	Vega Renovations LLC on or about December 12, 2020, at the site location of 2024 Collingwood Toledo, OH: The employer did not ensure that employees had been trained in the appropriate use of fall protection such as, but not limited to, the use of personal fall arrest systems or alternative measures while performing roofing installation from elevated surfaces. Employees were exposed to a potential fall from a height of near 20 ft. to the level below while performing roof installation activities without the benefits of fall protection in place.

29 CFR  1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels is not protected by guardrail systems, safety net system, or personal fall arrest system nor are employees provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):  a)	Vega Renovations LLC on or about December 12, 2020, at the site location of 2024 Collingwood Toledo, OH: The employer did not ensure employees were protected from falls while removing shingles and installing felt underlayment material atop a steep sloped residential type structure. Employees were exposed to a potential fall from a height of near 20 ft. to the level below while performing roof installation activities. No form of fall protection was in place.

29 CFR  1926.1053(b)(1): When portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access; or, when such an extension was not possible because of the ladder's length, then the ladder was not secured at its top to a rigid support that will not deflect, and a grasping device, such as a grab rail, was not provided to assist employees in mounting and dismounting the ladder.   a)	Vega Renovations LLC on or about December 12, 2020, at the site location of 2024 Collingwood Toledo, OH: The employer did not ensure the side rails of the 32ft. fiberglass extension ladder used onsite were extended at least three feet above the landing surface. Two employees were exposed to a near 20' fall to the level below while using the ladder to access the north side of the steep slope roof.
