Sylvania, OH: The employer did not have a plan or program to identify job site hazards which assists employees in maintaining a safe working environment. Such programs would support the employer and employees in identifying and avoiding potential hazards, such as falls during job site operations, decreasing the probability of employee injury and/or illness.

29 CFR  1926.503(a)(1):The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  a) Vega Renovations LLC on or about April 08, 2021, at the site location of 6117 Alexa Ln. Sylvania, OH: The employer did not ensure that employees had been trained in the appropriate use of fall protection such as, but not limited to, the use of personal fall arrest systems or alternative measures while performing roofing installation from elevated surfaces. One employee and Employer working from atop a residential structure roof were exposed to fall from height of 25 ft. while performing roof installation activities without fall protection in place.

29 CFR  1926.1053(b)(1):The employer did not ensure where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails  did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a) Vega Renovations LLC on or about April 08, 2021, at the site location of 6117 Alexa Ln. Sylvania, OH: The employer did not ensure the side rails of the fiberglass extension ladder used on site to access the second story roof were extended at least three feet above the landing surface. An employee accessing the second story of a residential structure roof was exposed to a fall from height of 19 ft. while using the ladder in this condition.
