29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a)	Vega Renovations LLC. On or about July 7, 2022, at the address of 9773 State Route 224, Findlay OH 45840: The employer did not ensure employees were protected from falls while performing roofing work activities atop a single-story residential structure with multiple roof pitches. Employees operating in this conditions were exposed to falls of 10'- 4" or greater to the ground below.   Vega Renovations LLC was previously cited for a violation of this Occupational Safety and Health Standard 1926.501(b)(13), which was contained in OSHA inspection number 1524251, citation number 1, item number 4 and was affirmed as a final order on August 5, 2021, with respect to a workplace located at 6117 Alexa Ln., Sylvania, OH 43560  Vega Renovations LLC was previously cited for a violation of this Occupational Safety and Health Standard 1926.501(b)(13), which was contained in OSHA inspection number 1506779, citation number 1, item number 2 and was affirmed as a final order on April 6, 2021, with respect to a workplace located at 2024 Collingwood, Toledo OH 43608

29 CFR  1926.20(b)(1):The employer did not initiate and maintain accident prevention programs as necessary to comply with this part:  a)	Vega Renovations LLC. On or about July 7, 2022, at the address of 9773 State Route 224, Findlay OH 45840: The employer did not have a plan or program to identify job site hazards which assists employees in maintaining a safe working environment. Such programs would support the employer and employees in identifying and avoiding potential hazards such as falls during job site operations, decreasing the probability of employee injury and/or illness. Employee working in these conditions were exposed to falls of up to 10'- 4" or greater.   Vega Renovations LLC was previously cited for a violation of this Occupational Safety and Health Standard 1926.20(b)(1), which was contained in OSHA inspection number 1524251, citation number 1, item number 1a and was affirmed as a final order on August 5, 2021, with respect to a workplace located at 6117 Alexa Ln., Sylvania, OH 43560  Vega Renovations LLC was previously cited for a violation of this Occupational Safety and Health Standard 1926.20(b)(1), which was contained in OSHA inspection number 1506779, citation number 1, item number 1a and was affirmed as a final order on April 6, 2021, with respect to a workplace located at 2024 Collingwood, Toledo OH 43608.

29 CFR  1926.503(a)(1):The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  a)	Vega Renovations LLC. On or about July 7, 2022, at the address of 9773 State Route 224, Findlay OH 45840: The employer did not ensure that employees had been trained in the appropriate use of fall protection such as but not limited to the use of personal fall arrest systems or alternative measures while performing roofing operations from elevated surfaces greater than six feet from the level below. Employees working in these conditions were exposed to falls of up to 10'- 4" or greater.
