29 CFR 1926.102(a)(1): The employer does not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:  a. Employees performing roofing work are not protected from eye injuries while using pneumatic nail guns.  The employer fails to assure effective protections against eye and face injuries as required by the standard, as demonstrated on December 9, 2020, when employees are working on a residential roof at Lot 178, Gainesville, Florida, without eye protection while using pneumatic nail guns and are exposed to eye injuries from flying debris or nails.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels is not protected by guardrail systems, safety net system, a personal fall arrest system, or an alternative fall protection measure as provided by another provision in paragraph (b) of this section:  a. Employees performing roofing work are not protected from fall hazards by the use of a fall protection system.  The employer fails to assure effective protections against falls as required by the standard, as demonstrated on December 9, 2020, when employees are working on a residential roof at Lot 178, Gainesville, Florida, exposed to a 19 feet fall hazard to the ground, in that they are not attached to a personal fall protection system.

29 CFR 1926.1053(b)(1): Where portable ladders are used for access to an upper landing surface and the ladder's length allows, the ladder side rails do not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a. Employees using ladders to access the roof are not protected from fall hazards by extending the ladder 3 feet above the landing.    The employer fails to assure effective protections against falls as required by the standard, as demonstrated on December 9, 2020, when 3 employees using ladders to access a residential roof at Lot 178, Gainesville, Florida, are exposed to fall hazards greater than 6 feet to the lower level in that the ladders do not extend 3 feet above the landing.    DWC Contracting, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.1053(b)(1), which was contained in OSHA inspection number 1240181, citation number 2, item number 2, and was affirmed as a final order on December 1, 2017, with respect to a workplace located at 13657 NW 10th Place, Newberry, FL 32669.  DWC Contracting, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.1053(b)(1), which was contained in OSHA inspection number 1117452, citation number 1, item number 2, and was affirmed as a final order on May 17, 2016, with respect to a workplace located at SW 36 Drive, Lot 196, Gainesville, FL 32608.  DWC Contracting, Inc was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.1053(b)(1), which was contained in OSHA inspection number 1004680, citation number 1, item number 2, and was affirmed as a final order on February 5, 2015, with respect to a workplace located at 7827 SW 79th Drive, Gainesville, FL 32608.
