29 CFR 1910.147(c)(1):The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: a) 3301 Columbus Blvd., Philadelphia, PA - Maintenance Shop - The employer failed to develop, document and utilize an energy control program to prevent employees from being exposed to hazardous energy sources, such as gravitational energy when performing vehicle service and maintenance exposing employees to struck-by and caught-in-between hazards. On or about November 17, 2021, an while performing maintenance on the fifth wheel of the Yard Jockey, an employee suffered crushing injuries when the employees arm was caught-in-between fifth wheel and the frame of a Yard Jockey.

29 CFR 1910.147(c)(4)(i):Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) 3301 Columbus Blvd., Philadelphia, PA - Maintenance Shop - Maintenance Shop - The employer did not develop hazardous energy control procedures for conducting maintenance activities, such as the replacement of hydraulic cylinder on a fifth wheel of a Yard Jockey Truck. On or about November 17, 2021, while removing a hydraulic cylinder of of the Yard Jockey, an employee suffered crushing injuries when the employees arm was caught-in-between fifth wheel and the frame of a Yard Jockey.
