29 CFR 1910.132(d)(2): The employer shall verify that the required workplace hazard assessment has been performed through a written certification that identifies the workplace evaluated; the person certifying that the evaluation has been performed; the date(s) of the hazard assessment; and, which identifies the document as a certification of hazard assessment. At a facility located at 600 Thilmany Road, Kaukauna, WI 54130; the employer did not certify in writing that a hazard assessment had been performed to evaluate the workplace hazards and the PPE those hazards necessitated when employees were changing the dust collector filters in the Lime Kiln Baghouse, exposing employees to airborne calcium oxide (lime) dust.

29 CFR 1910.134(d)(1)(iii): The employer shall identify and evaluate the respiratory hazard(s) in the workplace; this evaluation shall include a reasonable estimate of employee exposures to respiratory hazard(s) and an identification of the contaminant's chemical state and physical form. Where the employer cannot identify or reasonably estimate the employee exposure, the employer shall consider the atmosphere to be IDLH: At a facility located at 600 Thilmany Road, Kaukauna, WI 54130; the employer did not evaluate the respiratory hazards in the Lime Kiln Baghouse and did not evaluate a reasonable estimate of employees' exposure to respiratory hazards from airborne corrosive powdered calcium oxide (lime dust). Maintenance employees were exposed to airborne lime dust during the task of changing out filters in the Lime Kiln dust collector.
