All deficiencies such as listed shall be carefully examined and determination made as to whether they constitute a safety hazard:  On or about February 16, 2022 and prior, at an establishment located at 900 Thilmany Road, Kaukauna, Wisconsin; at Paper Machine 14 on the Dry End; the Shaw Box 0006150 overhead overhung crane utilizing the Shawbox DMR-3, S/N: 58-37177 Hoist #23A (North) was defective and not taken out of service.  Defects included but are not limited to:  the hoist brake continued to drift down after the operator released the controls exposing employees to stuck-by hazards.  The employer did not remove or tag the hoist out of service until repaired.

The employer did not remove and/or tag the hoist out of service until repaired.
