29 CFR 1910.219(d)(1):Pulley(s) with part(s) seven feet or less from the floor or work platform were not guarded in accordance with the requirements specified in 29 CFR 1910.219(m) and (o). On or about December 14, 2020, and times prior thereto, in the NE corner of the 5/8 building, the employer did not ensure the tail pulley of the 5/8 line conveyor was guarded exposing an employee to caught-in and amputation hazards.

29 CFR 1910.219(c)(4)(i):Projecting shaft ends did not present a smooth edge and end and projected more than one-half the diameter of the shaft without being guarded by nonrotating caps or safety sleeves. On or about January 13, 2021, and times prior thereto, the employer did not ensure that projecting shaft ends were guarded or less than one-half the diameter on conveyor systems, such as but not limited to in the NE corner of the 5/8 building, exposing employees to caught-in and struck-by hazards.
