29 CFR 1910.219(c)(2)(i):All exposed parts of horizontal shafting seven (7) feet or less from floor or working platform, excepting runways used exclusively for oiling, or running adjustments, shall be protected by a stationary casing enclosing shafting completely or by a trough enclosing sides and top or sides and bottom of shafting as location requires. On or about January 05, 2021, and times prior employees were exposed to a caught in hazard by not having a horizontal shaft guarded on: a) Re-reeling & Doctor machine in the production line. b) ACCRAPLY model AS12 in the production line. c) ACCRAPLY model AS12 in the production line.

29 CFR 1910.147(c)(4)(ii)(B):The energy control procedures did not clearly and specifically outline the steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy. On or about January 05, 2020, and times prior employees were exposed to a caught in hazard by not having specific procedures on the control of potentially hazardous energy when setting up the printing press: a) MPS on the production line. (Withdrawn) b) OMET on the production line. (Withdrawn) c) RP-10 on the production line. d) RP-8 in the production line. e) GH-8 in the duction line. (Withdrawn) f) GH-10-2 in the production line.
