29 CFR 1910.147(c)(5)(ii)(D):Lockout devices and tagout devices did not indicate the identity of the employee applying the device(s) The employer is failing to protect employees from caught-in, struck-by, and amputation hazards. This was most recently documented on at a worksite located at 4231 Clary Blvd, Kansas City , Missouri. Employees in the manufacturing area are using lockout tagout devices on Plate Machines, including but not limited to the Plate Machine known to the company as machine # 6. Lockout Tagout devices did not have a tag or any identification to show who applied the lockout tagout device. The lock and key were attached to the numerous plate machines including but not limited to Plate Machine # 6 and accessible to any worker in the area.

29 CFR 1910.147(c)(6)(i):The employer did not certify that periodic (annual) inspections of energy control procedures were performed. The certification should identify the machine or equipment on which the energy control procedure was being utilized, the date of the inspection, the employees included in the inspection, and the person performing the inspection. An examination of program elements determined that periodic inspections of energy control procedures were not accomplished and certified. Specifically, reviews were not conducted to determine if listed steps were still adequate, if any changes had occurred, or if employees had acquired the knowledge needed to effectively isolate energy sources.
