Employees were exposed to a fall of approximately 5 feet to the roof below.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. This violation occurred on January 27, 2021, where at least two (2) employees working on roof without fall protection were exposed to falls of approximately 20 feet to the concrete below. Tyler Roofing Company was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(1), which was contained in OSHA inspection number 1357395, citation number 01, item number 001 and was affirmed as a final order on April 12, 2019, with respect to a workplace located at 2815 WSW Loop 323, Tyler, Texas. Tyler Roofing Company was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(1), which was contained in OSHA inspection number 1136530, citation number 01, item number 001 and was affirmed as a final order on June 3, 2016, with respect to a workplace located at 419 WSW Loop 323, Tyler, Texas.
