On or about February 3, 2021, the employer did not ensure the operator of an American Fluid Power 2CP Flaring Machine, known as the CP2 Flaring Machine, was protected by one or more methods of machine guarding from the hazards created by the point of operation and moving and rotating parts of the machine; including but not limited too; the clamp and spindle.
