On or about February 29, 2024, an employee did not utilize equipment specific procedures to ensure energy control methods are in place prior to performing set-up procedures by exchanging dyes with-in the point of operation area [clamping area, die and tube stop] on a Hydraulic Flaring Machine, known as Big Blue.

29 CFR 1910.1030(g)(2)(i): 29 CFR 1910.1030(g)(2)(i): The employer shall train each employee with occupational exposure in accordance with the requirements of this section. Such training must be provided at no cost to the employee and during working hours. The employer shall institute a training program and ensure employee participation in the program. On or about February 29, 2024, the employer did not ensure that training was provided to a supervisor with occupational exposure or other potentially infectious materials when providing first aid to an injured employee. The employers blood borne pathogens programs assigns both first aid responders and supervisors to be trained in medical or first aid duties by their employers in accordance with 29 CFR 1910.1030(g)(2)(i).

29 CFR 1910.212(a)(3)(ii): The point of operation of machines whose operation exposes an employee to injury, shall be guarded. The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle. a. On or about February 29, 2024, the employer did not ensure the point of operation [clamping area, die and tube stop] of the Hydraulic Flaring Machine, known as Big Blue, was guarded to prevent the operator from having any part of his body in the danger zone during the operation cycle. Hycom Inc., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.212(a)(1), which was contained in OSHA inspection number 1665199, citation number 1, item number 1, and was affirmed as a final order on August 3, 2023, with respect to a workplace located at 374 5th Street, NW, Barberton, Ohio. Hycom Inc., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.212(a)(1), which was contained in OSHA inspection number 1513105, citation number 1, item number 1, and was affirmed as a final order on March 30, 2021, with respect to a workplace located at 374 5th Street, NW, Barberton, Ohio.
