(b) On or about February 3, 2021, an employee working on the first tier of self-supported tubular welded end frame scaffolding was not protected from a fall hazard of approximately seven feet in that the scaffolding was not fully planked.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used. Cross braces were used as a means of access:  (a) On or about February 3, 2021, at least two employees were accessing self-supported welded tubular end frame scaffolding by using the end frames and cross braces exposing them to a fall hazard of up to approximately thirteen (13) feet to the ground below.
