29 CFR 1926.200(g)(2): The design and use of all traffic devices, including signs, signals, markings, barricades, and other devices, for protection of construction workers did not conform to Part 6 of the MUTCD (incorporated by reference, see 1926.6): a) Jobsite located at 2601 Poplar Street, Philadelphia PA 19130: The employer did not provide advance warning to road users that a JLG 660SJ boom lift was on the roadway and what actions the road user must take to drive through the work zone. Employees working from the boom lift were exposed to struck-by and fall hazards of approximately 20 feet from the ground. Condition noted on Feb. 10, 2021.

29 CFR 1926.453(b)(2)(iv): Employee(s) were not standing firmly on the floor of the basket, were sitting or climbing on the edge of the basket, or were using planks, ladders, or other devices for a work position: a) Jobsite located at 2601 Poplar Street, Philadelphia PA: The employer did not ensure that employees working from a JLG 660SJ extendable boom lift were standing firmly on the platform of the lift. Employees were installing windows and were standing on the mid-rail of the boom lift as well as on a job-made wooden structure, thus exposing employees to fall of approximately 20 feet. Condition noted on Feb. 10, 2021.
