29 CFR  1926.1419(e):The signals used (hand, voice, audible, or new), and the means of transmitting the signals to the operator (such as direct line of sight, video, radio, etc.), were not suitable for the site conditions.       On or about February 25, 2021, employees were not adequately instructed in the use of hand signals for safe nonverbal communication between the drilling rig operator and the swamper, particularly in low light conditions.  Employee was exposed to the hazard of being caught-in or caught-between.
