(a) On or about February 24, 2021, at the blanker number four (4) machine (Drusula 1200S - Serial #10315), the employer did not insure lockout procedures specifically outlined steps for isolating/blocking feed rollers during the removal of jammed paper. This exposed employees to crushing hazards and amputation injury created by feed rollers ingoing nip point.
