29 CFR 1926.1053(b)(1):When portable ladders were used for access to an upper landing surface, the ladder side rails was not extended at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: On or about March 5, 2021, and at times prior thereto, employee operating Gorilla Ladder extension ladder that did not extend at least three feet above the upper landing surface, exposing employee to a fall hazard.

29 CFR 1926.451(f)(2):The use of shore or lean-to scaffolds is prohibited. On or about March 5, 2021, and times prior thereto, two employees were observed working on a temporary wooden platform shore style scaffold that was affixed to the forks of the lift via rope and lifted in the air via an aerial forklift and being placed against the structure of the stairwell for support.

29 CFR 1926.501(b)(1):"Unprotected sides and edges." Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. On or about March 5, 2021, and at times prior thereto employees engaged in structural demolition activities near unprotected elevated edges were not provided fall protection and exposed to an unprotected fall hazard of approximately 25 feet. Pearl Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(1), which was contained in OSHA inspection number 1300264, citation number 1, item number 1, and was affirmed as a final order on 05/10/18, with respect to a workplace located at 1721 Community Avenue, McKinney, TX 75071.
