On or about March 10, 2021, an employee working in an excavation was exposed to struck by and crushed by hazards when water accumulated in the excavation and there was no cave-in protection.

29 CFR 1926.651(c)(2):A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees: On or about March 10, 2021, an employee working in an excavation over 4 feet in depth was exposed to a struck-by hazard when they did not have a safe means of egress. UST Systems, Inc. was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1350941, citation number 1, item number 2a and was affirmed as a final order on December 6, 2018, with respect to a workplace located at 4611 East Martin Luther King Jr Blvd Austin, TX 78721.
