1926.21(b)(2) The employer shall instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his work environment to control or eliminate any hazards or other exposure to illness or injury. (a) 400 Lehigh St. Allentown, PA., Trench: On or about March 25, 2021 during excavation operations, not all employees were protected from cave-ins by an adequate protective system while working in the 11-foot, deep-excavation.
