29 CFR  1926.451(c)(2)(v):The employer used forklifts  to support scaffold platforms without ensuring that the entire platform was attached to the fork and that the forklift was not moved horizontally while the platform was occupied:  The employer is failing to protect employees from fall hazards while working from forklift supported scaffold platforms on a residential construction site. This was most recently observed at 5420 Sheridan Rd, Bellevue, Nebraska on March 31, 2021 where the employer has employees using a supported scaffold platform elevated by the forks of a forklift as a work surface without ensuring the  platform was attached to the forks.

29 CFR  1926.451(g)(4)(i):Guardrail systems were not installed along all open sides and ends of platforms.  The employer is failing to protect employees from fall hazards while working from elevated platforms without fall protection on a residential construction site. This was most recently observed at 5420 Sheridan Rd, Bellevue, NE 68123 on March 31, 2021 where employees were using a work platform that was missing a top-rail and elevated by the forks of an all terrain forklift approximately 20 feet above the ground.

29 CFR  1926.1052(c)(1):Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, were not equipped with at least one handrail and one stair rail system along each unprotected side or edge:  The employer is failing to protect employees from fall hazards while utilizing the stairway inside a new residential construction home. This was most recently observed at 5420 Sheridan Rd., Bellevue, Nebraska on March 31, 2021 when employees were utilizing a stairway they constructed as means of access and egress from the basement through the first floor and onto the second floor.  The stairway did not have a stair rail system installed for the  unprotected sides.

29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system:  The employer is failing to protect employees from fall hazards while performing residential roofing activities. This was most recently observed at 5420 Sheridan Rd, Bellevue, Nebraska on March 31, 2021 where the employer has employees installing roof sheathing on a new residential home without fall protection where the ground to eave was over 20 feet.   Ramirez Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1926.501(b)(13)), which was contained in OSHA inspection number 1369536, citation number 2, item number 1 and was affirmed as a final order on May 15, 2019, with respect to a workplace located at 17940 Camden Ave, Elkhorn, NE 68022.
