29 CFR 1928.57(a)(6)(iii): Stop engine, disconnect the power source, and wait for all machine movement to stop before servicing, adjusting, cleaning, or unclogging the equipment: a) Opal Foods, LLC, 9575 WCR 73, Roggen, CO 80652: On and before April, 15, 2021, the employer did not ensure that employees were to wait for all machine movement to stop before servicing and or conducting maintenance. The employer assigned a worker to clean manure from the Pit Manure Belt conveyor but did not ensure it was isolated and inoperative. This condition exposed employees to hand and finger amputations.

29 CFR 1928.57(a)(6)(v): Lock out of electrical power was not conducted prior to performing maintenance or service on farmstead equipment: a) Opal Foods, LLC, 9575 WCR 73, Roggen, CO 80652: On and before April, 15, 2021, the employer did not ensure that employees cleaning the Pit Manure Belt were protected from injury in that the equipment was not isolated from the energy source and rendered inoperative prior to starting maintenance activities and worker sustained a finger amputation.

29 CFR 1928.57(a)(11):Where a moving machinery part presented a hazard during servicing or maintenance, the engine was not stopped, the power source was not disconnected, and all machine movement was not stopped before servicing or maintenance was performed, except where the Employer could establish the following in 1928.57(a)(11)(i-iii): a) Opal Foods, LLC, 9575 WCR 73, Roggen, CO 80652: On April, 10, 2021, a maintenance employee unclogging manure in the North Layer on the Pit Manure Belt sustained a finger amputation. The power source was not disconnected, and all machine movement was not stopped before maintenance was performed.
