On or about April 26, 2021, employees were exposed to a fall of approximately 24 feet while performing roofing activities on a residential home located at 9662 Diamond Ridge Cr., Canal Fulton, Ohio 44614. The employees were using a Equiptor RB 4000 as a scaffold platform to reach upper levels of the home and the employer did not ensure that ladders were not used to increase the working level height of the employees.

The employees were using air powered nail guns and the employer did not ensure that they were wearing protective safety glasses.
