29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  On or about December 18, 2025 , employees were exposed to a struck by hazard while performing roofing activities on a residential home located at 3980 Walnut Wood Way, Uniontown Ohio 44685.  Employees were working on the roof with tools and materials that could have fallen to the lower level and the employer did not ensure that the employees on the lower level were wearing protective helmets.

29 CFR  1926.102(a)(1): Employees did not use eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:   On or about December 18, 2025, employees were exposed to a struck by hazard while performing roofing activities on a residential home located at 1948 Hickory Ave NE, Massillon, Ohio 44646.  The employees were using battery powered saw, air powered nail guns, and hand tools  and the employer did not ensure that the employees were wearing protective safety glasses.  Miller Roofing & Exteriors Limited was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFE 1926.102(a)(1), which was contained in OSHA inspection number 1830807, citation number 1, item number 1, and was affirmed as a final order on 8/8/2025, with respect to a workplace located at 1948 Hickory Ave NE, Massillon, Ohio 44646.  Miller Roofing & Exteriors Limited was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFE 1926.102(a)(1), which was contained in OSHA inspection number 1527464, citation number 2, item number 1, and was affirmed as a final order on 6/16/2021, with respect to a workplace located at 9662 Diamond Ridge Cir, Canal Fulton Ohio 44614.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   On or about December 18, 2025, employees were exposed to a fall of approximately 17.8 feet while performing roofing activities on a residential home located at 3980 Walnut Wood Way, Uniontown Ohio 44685. The employees were working on all areas of the roof and the employer did not ensure that they were using fall protection.  Miller Roofing & Exteriors Limited was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFE 1926.501(b)(13), which was contained in OSHA inspection number 1830807,  citation number 1,  item number 2, and was affirmed as a final order on 8/8/2025, with respect to a workplace located at  1948 Hickory Ave NE, Massillon, Ohio 44646.
