29 CFR  1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury:   (a) Jobsite located at 456-500 N 5th St, Philadelphia PA:  The employer did not ensure that one employee, operating a JLG 660SJ extendable boom lift, had not been instructed on the applicable procedures and/or regulations to minimize those hazards while performing this work. Condition noted on May 12, 2021.

29 CFR 1926.503(a)(1): The employer did not train each employee who might be exposed to fall hazards to enable them to recognize fall hazards and the procedures to be followed in order to minimize these hazards:   (a) Jobsite located at 456-500 N 5th St, Philadelphia PA: The employer did not provide an effective training program for: 1) employees performing window installations at an unprotected wall opening and 2) employees working near large floor openings on the roof , thus exposing the employees to 20 and 10 foot fall hazards. 29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a) 456-500 N 5th St, Philadelphia, PA: The employer did not ensure that one employee, installing a large window at an exterior wall opening, was protected by a guardrail or other fall protection measures, thus exposing the employee to a potential fall height of 20 feet.   Condition noted on or about May 12, 2021 @ 215pm.  b) 456-500 N 5th St, Philadelphia, PA: The employer did not ensure that two employees, performing rough framing at the unprotected edges of 2 exterior wall openings, were protected by a guardrail or other fall protection measures, thus exposing employees to potential fall heights of 20- and 30-feet.   Condition noted on or about May 11, 2021 @ 1114am.    A Squared Construction LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1491777, citation number 1, item number 1 and was affirmed as a final order on 2/20/2021, with respect to a workplace located at 2035 E Lehigh Ave, Philadelphia, PA.  A Squared Construction LLC DBA A Squared Framing was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1394224, citation number 1, item number 1 and was affirmed as a final order on 5/29/2019, with respect to a workplace located at 1522, 1524, and 1526 North Philips Street, Philadelphia, PA.   A Squared Construction LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1434359, citation number 1, item number 1 and was affirmed as a final order on 1/16/2020, with respect to a workplace located at 1524 N. Philip St., Philadelphia, PA.
