29 CFR  1926.501(b)(4)(i):Each employee on walking/working surfaces was not protected from falling through holes (including skylights) more than 6 feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around the floor hole.  (a)Fiegen Construction Co. at 26 1st Avenue Southwest in Watertown, SD 57201: On or about and at times prior to May 12, 2021, the employer did not ensure that employees were protected from fall hazards in that all floor holes in the core floor panels were not appropriately covered prior to employees performing work in the area. In areas where floor holes were not covered, employees did not use personal fall protection or a guardrail system when working in the area.  An employee fell approximately 13 feet through an uncovered floor hole resulting in serious injuries.    Abatement Note:  Abatement certification and documentation is required for this item (see enclosed "Certification of Corrective Action Worksheet").
