29 CFR  1926.25(a): During the course of construction, alteration, or repairs,  debris was not kept cleared from work areas, passageways, and stairs, in and around buildings or other structures:  a) Lot 51-56 Anthem St, Ranson WV: On or about May 11, 2021 and times priors, employees were working installing framing on a row of residential homes. Lumber debris with protruding nails was scattered throughout the site exposing employees to trip and puncture/impalement hazards.

29 CFR  1926.501(b)(13):Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501(b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:   a) Lot 51-56 Anthem St, Ranson: On or about May 11, 2021 and times prior, an employee was exposed to a fall of 23 feet frame to ground, while sitting on the frame nailing in other frame sections on a new residential townhome. The employee was not utilizing a fall protection system or fall arrest harness.  b) Lot 51-56 Anthem St, Ranson: On or about May 11, 2021 and times prior,  an employee was exposed to a fall of 15 feet floor to ground, while leaning out a window opening to nail in insulation pieces on a new residential town home.   The employee was not utilizing a fall protection system or fall arrest harness.   c) Lot 51-56 Anthem St, Ranson: On or about May 11, 2021 and times prior, an employee was exposed to a fall of 15 feet 4 inches floor to ground, while standing at the floor edge of a new residential townhome directing a fork lift. The employee was not utilizing a fall protection system or fall arrest harness.   d) Lot 51-56 Anthem St, Ranson: On or about May 11, 2021 and times prior, an employee was exposed to a fall of 15 feet,  while standing at the 2nd story left corner of a residential townhome assisting another employee with measurements between two townhomes.   The employee was not utilizing a fall protection system or fall arrest harness.   e) Lot 51-56, Anthem St, Ranson:  On or about May 11, 2021 and times prior, an employee was exposed to a fall of 13 feet 10 inches frame to ground, while standing on a section of the frame that was sticking out from a residential townhome, assisting another employee take measurements between two town homes. The employee was not utilizing a fall protection system or fall arrest harness.

29 CFR  1926.1053(b)(1):Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder.    a) Back of home: On or about May 11, 2021 and times prior, employees were exposed to falls of 13 feet while using a Werner extension ladder to gain access to the second floor of a new residential townhome. The ladder was approximately 1 foot below the edge of the floor.
