29 CFR 1926.501(b)(4)(ii):Each employee on a walking/working surface shall be protected from tripping in or stepping into or through holes by covers. a) On or about 05/27/2021, at the NYSDOT Route 31 bridge in located in Knowlesville, the town of Ridgeway, New York, the employer did not cover a hole measuring 12 inches long and 6 inches wide. Employees performing demolition work on the bridge surface were exposed to falling in and through the hole opening. NO ABATEMENT DOCUMENTATION REQUIRED
