29 CFR  1926.102(a)(1):The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  a. On or about June 2, 2021, at the above addressed jobsite, an employee using pneumatic nail guns was not wearing safety glasses, thereby exposing employee to hazards associated with eye injuries.  The P & D Construction, Inc. was previously cited for a violation of this occupational safety and health standard 29 CFR 1926.102(a)(1), which was contained in OSHA inspection number 1452693, citation number 1, item number 2 and was affirmed as a final order on April 8, 2020, with respect to a workplace located at Lot 1 - Sage Brush Lanecountry Pond Estates Mokena, IL 60448.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b).  a.  On or about June 2, 2021, at the above addressed jobsite, an employee performing carpentry operations on the outside top plate while installing roof rafters, eighteen (18') feet two (2") inches above the ground, was not protected from falling, thereby exposing an employee to fall hazards.  P & D Construction, Inc. was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1214553, citation number 1, item number 1 and was affirmed as a final order on April 12, 2017, with respect to a workplace located at 815 S. Vine St., Hinsdale, IL.  P & D Construction, Inc. was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1452693, citation number 2, item number 1 and was affirmed as a final order on April 08,2020, with respect to a workplace located at Lot 1- Sage Brush Lanecounrty Pond Estates, Mokena, IL.
