29 CFR 1910.119(j)(5): The employer did not correct deficiencies in equipment that were outside acceptable limits (defined by the process safety information on paragraph (d) of this section) before use(a): On or about June 9, 2021, at the location of 4719 NAVARRE RD. SW, Canton, OH 44706, the employer failed to correct deficiencies in equipment that were outside acceptable limits: a) The employer failed to correct deficiencies in the CTI, GG-NH3 ammonia monitors (SN's 000187405 - HTA, 000187404 - SC4, 000187403-Mezzanine, and 000187406 mezzanine) that were outside acceptable limits, before use. The monitors were installed and have been in use since September 2020 and required calibration March 2021. b) The employer failed to assure that the recently replaced CTI, GG-NH3 ammonia monitors were compatible with the existing engine room PLC (Allen Bradley - 1500) control panel that integrated the ventilation, ammonia monitors, and emergency shut off. The control panel was not operational. Employees were exposed to ammonia vapors and ammonia is a hazardous chemical that can cause respiratory irritation, unconsciousness, and death.

On or about June 9, 2021, at the location of 4719 NAVARRE RD. SW, Canton, OH 44706, the employer did not perform appropriate checks and inspections to assure that equipment was installed properly and consistent with design specifications and the manufacturer's instructions: a) The employer failed to correct deficiencies in the CTI, GG-NH3 ammonia monitors (Serial Number's 000187405 - HTA, 000187404 - SC4, 000187403-Mezzanine, and 000187406-Mezzanine) that were outside acceptable limits before use. SW, Canton, OH 44706, process safety information was not updated to reflect changes in the process safety information: a) The CTI, GG-NH3 ammonia monitors (Serial number's 000187405 - HTA, 000187404 - SC4, 000187403-Mezzanine, and 000187406 -Mezzanine) were installed and have been in use since September 2020. The process safety information reflected the use of Greer ammonia sensors that were removed in September 2020. b) The employer failed to assure that the recently replaced CTI, GG-NH3 ammonia monitors were compatible with the existing engine room PLC (Allen Bradley - 1500) control panel that integrated the ventilation, ammonia monitors and emergency shut off. The control panel was inoperable. Employees were exposed to ammonia vapors and ammonia is a hazardous chemical that can cause respiratory irritation, unconsciousness and death.

29 CFR 1910.305(a)(2)(x): Flexible cords and cables were not protected from accidental damage (for example, by sharp corners, projections, and doorways or other pinch points): On or about June 9, 2021, at the location of 4719 NAVARRE RD. SW, Canton, OH 44706, in the cheese incubator room, a blue temporary electrical cord was run through a freezer door and exposed to sharp corners of the doorway and door. The cord was energizing temporary exhaust fans exposing employees to electrical shock hazards.
